What is Breast Lift Surgery?

A breast lift, also known as mastopexy, is a surgical procedure designed to raise and reshape deflated breasts that have lost a natural more youthfully appealing contour. As we age and our bodies undergo natural changes whether it is through natural breast development, weight fluctuations, or from a pregnancy followed by breastfeeding, the breast shape can become compromised. As the breast tissue and breast skin envelope expand and then contract the ligaments holding the breast stretch. In the end, the weight of the breast tissue with some help by gravity can cause breasts to then descend and elongate, losing their youthful shape and firmness. 

To address the stretched out skin envelope and resulting descent of the breast tissue, during a mastopexy/breast lift procedure, excess skin is often removed through a special plastic surgery pattern that is tailored for each patient. The remaining breast tissue is reshaped and repositioned to recreate a more youthful shape and projection. The power of the procedure is that the nipple-areola complex is also repositioned to a higher, more youthful position.

This procedure can be performed alone or in combination with breast augmentation with either fat grafting or implants to achieve your desired breast size and shape.

What is Breast Lift Surgery Like?

Consultation: Dr. Kaufman will take a complete medical and surgical history with a detailed breast history in regards to previous breast surgery, family history of breast disease or cancer and recent breast imaging results. A series of measurements and medical photography will facilitate surgical planning to address your concerns and discuss your goals followed by a recommendation of the various surgical options to maximize your result.

Customized Planning: Together, you will decide on the extent of lifting needed and any additional procedures to be performed to help you reach your goals. A second consultation to review options and the surgical plan as well as post-operative expectations is always welcome prior to your surgical date.

Surgery Day: The surgical procedure is typically performed under general anesthesia and takes between 1.5 to 4 or more hours depending on the complexity of the case. Expect to arrive 1 to 2 hours prior to surgery with a friend or family member to keep you company and help get you home safely after surgery. You will be greeted by friendly pre-operative staff and nurses that will check you in and start the intravenous line. Shortly thereafter you will have a chance to meet the anesthesia team and Dr. Kaufman will greet you as well. He will answer any remaining questions regarding your surgery and then mark you while you are sitting up. After the procedure, expect to recover for at least an hour or 2 before going home. The post-operative medications will be sent to your pharmacy before leaving for home. All and all it is a long day but well worth the effort. 

Incision Patterns: Dr. Kaufman will design a pattern unique to your breast shape and size that will best achieve your goals. He does not use a one size fits all pattern, but adjusts and contours standard patterns for the best results. Most patients will have a combination of periareolar (around the nipple) and vertical incisions to complete a vertical mastopexy pattern. Additional horizontal incisions placed underneath the breast along the breast crease or inframammary fold can be added for patients with larger amounts of excess skin envelope.

  • Periareolar (around the areola for a 1 to 2 cm lift)
  • Vertical Mastopexy (around the areola with vertical extension to achieve 2 to 5 cm of lift)
  • Anchor/ Inverted-T (around the areola, vertical component, and horizontally along the breast crease for greater than 5 cm of lifting)

Tissue Reshaping and Repositioning of the Nipple: Once the skin envelope has been modified with a pattern, the breast tissue is then lifted and reshaped to its optimal position on the chest wall allowing for both improvement in projection of the breast and a more youthful shape. Finally, the nipple-areola complex is repositioned and inset into its new location.

Breast Lift Post-Operative Recovery

We have found that patients undergoing breast surgery and breast lift have minimal to almost no post-operative pain. Prior to starting your surgery, you will receive an effective pre-operative combination of pain medications to block multiple types of pain receptors. In addition, Dr. Kaufman will perform a local anesthetic block of the nerves innervating the breast tissue. The goal is that your body and brain will not experience or perceive pain during the surgery allowing for a faster recovery.

After your breast lift surgery:

  • Expect some swelling, bruising, and mild to moderate discomfort in the first 24 to 72 hours
  • You will be followed closely in the postoperative period with at least 1 to 2 visits in the first week followed by weekly visits to monitor your progress
  • Sutures and surgical drains are removed in the office on follow-up
  • Wear a surgical support bra as directed by Dr. Kaufman
  • Gradually return to normal activities within the first week
  • May return to strenuous exercise and heavy lifting within 3-6 weeks

Dr. Kaufman and his surgical team will provide comprehensive aftercare guidance and support throughout your recovery process. The final breast results are not usually seen for a few months after surgery for all swelling and healing on the incisions to resolve.

Initial results are visible immediately after surgery, but final results develop over several months as swelling subsides and tissues settle. Most patients see a significant improvement in breast shape and position but final results take between 6 to 9 months.

How long do breast lift results last?

Results from a breast lift can be long-lasting but are nonetheless variable based on an individual's tissue laxity, prior surgical history, overall health and changes in weight as well as through the natural aging process. It is not uncommon to require revisional surgery for small adjustments after the initial surgery as well as re-lifting after a few years as natural aging and gravity will continue to affect your breasts over time.

Frequently Asked Questions

Mastopexy procedures and lifting the breast can give the appearance of increasing the breast volume by enhancing overall projection and consolidating volume into a smaller skin envelope. The total volume of the breast tissue itself does not change, unless adding volume through a fat grafting procedure or using a breast implant to increase volume and projection. If you desire considerably more volume a combined procedure can best achieve this goal, but we have found that often the desired effect can be achieved without needing to use implants.

This is my favorite question regarding surgery. Each incision is resulting in tissue healing and scar formation. Scaring is a natural process and our bodies' way of recovering from the surgical incisions. So, scarring is inevitable but typically fades and becomes less noticeable over time. If you have a tendency towards poor healing or scar formation we can address that prior to surgery.

Most women can breastfeed after a breast lift, but it's important to discuss this with Dr. Kaufman if you plan to have children in the future.
